Complaint Form to Request for Feedback on Local Authorities Delivery System

Dear PAM Member

After the launching and implementation of One Stop Centre (OSC) and Certificate of Completion and Compliance (CCC) since 2007, many complaints have been received from various parties dealing with the Local Authorities. The complaints are generally related to inconsistencies and discrepancies of OSC and CCC by different Local Authorities.

Therefore, we enclose herewith a Complaint Form for you to highlight issues you have with the Local Authorities with whom you have been liaising. The purpose of this survey is to gather feedback from Members in order for PAM to forward the encountered problems to the Local Government Department of the Ministry of Housing and Local Government for them to take necessary action to improve the system.
